Needs List: What We Need from You

• Copy of photo ID and social security card
• Income information
  • Pay stubs—One month's worth
  • W2 and/or 1099 forms for previous 2 years
  • Tax returns—previous 2 years with 2nd page signed
  • Pension/award letters (i.e., Social Security)
• Assets—most recent (all pages must be submitted, even blank ones)
  • Bank Statements—two months
  • Retirement Statements—two months
  • Mutual Fund Statements—two months
• Current mortgage information
  • Most recent statements of all mortgages
• Homeowners insurance information
  • Company, agent name, policy number, and Phone number
• Employment
  • Name, address, and phone number, for past 2 years
• Residence information
  • Address for previous two years. If you have been renting, you will need to provide the name, address, and phone number of landlord and copy of rental or lease agreement.

If Any of These Apply, Please Provide

• Bankruptcy—if filed in last seven years, a copy of the discharge and a complete copy of the bankruptcy filing with the list of creditors.
• Real Estate owner—address(es), account number(s), payments, remaining mortgage balance(s), and rental agreements
• Divorce—complete divorce decree to verify child support and/or alimony and maintenance payments
• Self-employed—two years corporate and personal tax returns (include P & L's)
• Not a U.S. citizen—green card information
• Commision income (more than 25% of income)—two years personal tax returns, including schedules and attachments
• Gift funds—donor's bank statement or a bank letter demonstrating the donor's ability to provide gift funds.
